Dancer Registration

Please fill out the form completely to register for the 2009 Dance Marathon. The registration has two parts: a form for signing up for the Marathon and a Medical Emergency Form. At the end you will be asked to read and complete waivers, which must be mailed to us in order for you to participate. You will also need to send in a $25 registration fee; mailing information for the waivers and the fee will be given to you once you complete the registration. If you have any problems with the system, please email our tech director so that he may help you. Thanks for your help!

Team Captain


Pledges by dancers' friends and families contribute to the largest portion of donations we recieve all year, so we need your help! Give us the names and address of up to 10 people (ONLY FIVE NAMES ARE REQUIRED) you know would be willing to help so we can make our goal of $100,000 possible; YOU are responsible for raising at least $150. You may provide e-mail addresses instead of mailing addresses, which is preferred, to help save on paper costs and we'll send a personalized e-mail to the donor on your behalf. We know - it sounds like a lot, but it's for the kids!
